INTRODUCTION

Under the supervision of Health Homes 2 Lead Physician, the Ambulatory Care Clinical Pharmacist fills three principal roles: (1) provider of direct patient care, with an emphasis on Medication Therapy Management and patient education on topics of medication safety, side effects and interactions; and (2) collaborator with Health Homes 2 clinical staff; 3) quality leadership on issues of patient safety, polypharmacy, high-risk medications especially in the elderly, and chronic pain medications. The Ambulatory Care Clinical Pharmacist supports the Health Homes 2 team and the implementation of a comprehensive chronic care model, patient care plans and assists in identifying optimal approaches to team-based patient care, including care management, patient education, and self-management support. The Ambulatory Care Clinical Pharmacist participates in quality improvement, patient safety, and project activities as they pertain to the Health Homes 2 program.

ESSENTIAL FUNCTIONS

  • Collaborates in providing medication therapy management to patients in conjunction with a multidisciplinary care team, including primary care providers, nurses, case managers, medical assistants and other health center staff.
  • Serves as a consultant to providers on clinical pharmacy issues.
  • Collaborates within the team-based, Patient-Centered Medical Home and Health Homes 2 model to provide chronic disease management services, including patient education and self-management support.
  • Designs, organizes, conducts, and evaluates the efficacy of individual and group learning and support sessions for patients with chronic conditions.
  • Provides Health Homes 2 program-wide leadership on issues of patient safety including chronic opioid use, polypharmacy, and high-risk medications especially in the elderly and children.
  • Collaborates with the Patient Safety Committee, Quality Improvement Working Groups, and other committees as assigned.
  • Identifies the need for quality monitoring and reporting related to the safe prescription of medications.
  • May accompany Health Homes 2 team members on outreach visits to include hospital, ER or home visits.
  • Shall attend Unity's Task Force and/or Pharmacy and Therapeutics committee upon request.
  • Supervise pharmacy students during their Unity rotation.
  • Performs other duties as assigned.

QUALIFICATIONS

· Doctor of Pharmacy degree (PharmD) from an accredited U.S. School of Pharmacy.

· Graduate of a PGY-1 or PGY-2 pharmacy residency program, ideally with a concentration in Ambulatory Care.

· Eligible for licensure as a pharmacist in the District of Columbia.

· Experience working with low-income populations.

· Strong interest in addressing health outcomes disparities.

· Bilingual in Spanish or Amharic a plus.

· Must commit to becoming a Certified Diabetes Educator within the two years of employment.
