Job Description



PRN/On Call



Under the supervision of the Clinical Director, the nursing assistant is responsible for providing basic nursing care, to include activities of daily living, non-medicinal dressing changes, and support to the patients.


· Measures and records vital signs to include: temperature, pulse, respiration, blood pressure, height, and weight.

· Performs admission procedures as directed.

· Completes the admission/intake forms.

· Regularly notifies the staff nurse of any changes in the patient's condition and/or abnormal vital signs.

· Assists the patients in self management and maintaining a clean environment by: supervising patients in making their beds, storage of clothing, keeping bedside table clean and neat, and activities of daily living.

· Observes universal precautions and uses proper techniques for all treatment and dressing changes, implementing appropriate infection control measures.

· Assists patients unable to perform tasks independently as needed.

· Assists in getting patients to meals.

· Provides coverage at the desk on the second floor as needed.

· Assists in getting patients to scheduled appointments.

· Assists the patient in preparing for appointments.

· Accompanies the patient to scheduled appointments as necessary.

· Completes the daily task assignment.

· Assists with discharge procedures.

· Performs other related duties and responsibilities.


· High School diploma or GED.

· Completion of a Certified Nursing Assistant program preferred.

· CNA preferred.

· Minimum of one year of experience in an acute care/long term care setting.

· Current CPR certification.


· Ability to pass a UHC Nursing Assistant screening test.

· Ability to communicate effectively orally and in writing.

· Experience with special needs population (i.e., homeless, HIV+, medically indigent, etc.).
